Does anyone recognize this door hinge?

Hello:

I'm sorry if this is the wrong forum to post in... but I figured carpenters have probably spent their fair share of time looking at multitudes of hinges.

I'm wondering if anyone recognizes this kind of hinge. I can't find it anywhere. They're on our bifold closet doors and we're looking to replace them in a satin nickel finish, but don't want to mess with puttying over old holes since we already painted the doors. Not a single hinge I've found matches the placement of the screw holes. The brand is Eagle, but there's various Eagle companies that sell hinges. I've emailed a few and am waiting for a response.

Took less than a minute with Google, "Bifold door hinge" Images tab, and Bob's your uncle!
