Does an electrician need to know the actual dimensions of a wall sconce before he installs an electrical box for it in the wall?

Usually not. Most lighting fixtures such as sconces are designed to mount in a standard lighting fixture box. Typically there's a metal plate that goes between the box screw holes that the fixture attaches to.
