Why not ask the office that gave you that paper?

I'm confident in my ability to remove the wall and relocate the switches, but I'd like to make my next visit to the permit office as professional as possible. It's clear that my inexperience with submitting permit applications will already be very obvious to those that will decide yes or no. I don't wish to need to make multiple trips if it turns out the information I needed to add was not difficult to obtain but requires observing something at home.

That sounds to me like you were given an inappropriate form or the township is lazy. Call them on the phone to confirm whether they really expect a homeowner to fill this out and, if so, have them specify exactly what they want, given that you are only moving switches. Just because you are not a licensed electrician does not mean you should be put at a disadvantage or intimidated. I've dealt with a number of small offices like that and frequently the clerks don't really know what they are doing.

That info is about the service entrance equipment (your main panel)and the transformer(pole or pad mount)supplying it. It has nothing to do with internal wiring. You either have the wrong form or you don't need to worry about it.

Are you sure this isn't in an area that says "for office use only"? Becaus that's what it looks like to me. There is no way you would need most of this information for a simple interior wiring job. Call them to check-while it may not always seem like it, they are there to HELP you. I find approaching bureaucrats with a friendly attitude, and playing a little stupid, almost always works wonders.
