beginner sewing machine problem

I don't know a word for it, but I keep ending up with three threads coming up from the bottom instead of one. I pulled the shuttle (? the metal thing that holds the bobbin) out, then removed, checked and replaced the (same) bobbin.

I've done this 3 times. I checked my upper thread, and it's all int he right place - I was sewing fine until my needle shifted out of position and was stuck crooked through the 'plate' and out of it's 'holster'.

Anyone know why my thread might be doing this?

There are no 'locks' (extra long stitches) on either side, and the stitches look fine except that there are three threads on the bottom a few inches into the stitch.

This is a brother XL 5130 that I got at walmart 7-8 years ago and use very rarely- as you can probably tell.
