adding expansion to home over exterior basement exit

Not sure if this is even the right forum . . . . . . We have contemplated an addition to the rear of our home that would increase the size of our kitchen, etc. One potential headache in doing this is that right below our kitchen door is an exterior exit from the basement, with concrete stairs that run parallel to the rear wall/foundation of the house. The house is an older colonial and it seems to me that this was maybe once a bilco door type stairwell, but it was changed to allow the door from the kitchen above and a bluestone patio. What are the options in dealing with this exit/ stairwell if we want to push out the back of the house? How problematic is this?
