Removing wood fireplace mantel/surround?

We'd like to replace our builder-grade fireplace mantel/surround with a better one. Mainly, we're wondering how we remove it from the wall without ripping the drywall to shreds? We know to score all around it with a utility knife, but where do we find/get to the nails/screws that are holding it to the wall?

Is it a prefabricated mantel or a custom fabricated assembled on site mantel? Pictures may help.

You may not be able to remove it without sacrificing drywall and paint. Pictures, please.
