LOOKING for: never fail peanut butter fudge

I was given this recipe several years ago and have lost it. It was very easy: contained sugar, peanut butter, milk, butter, vanilla and cocoa (optional). All ingredients went into pan, brought to a rolling boil and allow to boil for exactly one minute. Remove from heat and pour into greased dish.
