This will be the last time I try to add a pic. If it works the question is: Is this fixable or do we get a different concrete Co. to come out and re-do it?

I cannot believe that would be "fixable" in any way that would be acceptable. Hopefully you haven't made your final payment to the contractor. I would withold that and apply it to pay another contractor to come in, take it out, and re-do it.
