I have EVERYTHING, including all contact and insurance info, listed on one sheet of paper on the computer that is updated as needed. One copy goes on the fridge just in case of 911 call/visit, one goes into glove compartment and one reduced size in wallet.

I take a copy to every dr. visit as they seem to want updated info more often. When prior surgeries, current meds, etc. are requested I just write "see attached". It's all there and saves trying to remember everything and writing it all down, again.

Just like wantoretire_did, I keep an updated list of all medical information in my purse for myself and for my husband. I can hand this to the doctor to copy. I also keep a copy of our living wills.

I have an awesome medical alert bracelet which is actually a USB flash drive, I put it in my computer and fill out all the information or as much as I want to including a picture, any time something changes like a medication I can easily update it. Prior to a visit with a new doctor I just put the USB drive in the computer and hit copy to get a complete copy of everything on the drive.
I have had so many doctors tell me they had never seen such a great file and they all want to know how I did it. I just point to my bracelet. It never fails they want to see it and get the website for it. They want one for their own family and for patients.

I have probably sold a lot of these bracelets.

With as many medications as I take it is worth it just to keep an updated list easily available. Much better than the metal one I had before because they couldn't fit everything on the engraved ones.

Are you recommending this flash drive device instead of a Medic Alert bracelet? I'd be concerned with the ability of the people who work in most medical facilities to handle something like this that's out of the ordinary. Not infrequently, PCs in such places block the opening or connection of non-organization-wide files and programs.

The Medic Alert system is familiar to most health care providers and first responders.

Off topic but boy did I get a shock at a checkup with a new doctor/facility. For the first time in my life I was not asked for the date of my last menstrual period. I guess I am now officially OLD...

I am so darn old school, I have everything in a note book.

I do update it constantly, and take it with me to the doc.

Love wantoretires idea, and did start such a while ago, but somehow don't manage to update it as well as my notebook.

PS, instead of an i care bracelet I wear a road id, like the sport model in the link below. I wear it when I ride my bike, and even on road trips in my car.
Mine has the phone numbers of my kids and my doc as well as my full name on it.

If I'm not going to a new physician, I don't list any surgery/procedure older than 10 years. I put all that in my records in earlier visits and they already have those on file.

I also keep everything in one sheet on the computer and update it before each doctor's visit and give it to them.

I carry a USB drive with as mush of my medical history as I have including mammogram files and insurance policy for house and car. In addition to that I wear a bracelet advising to look on my wallet for further info on allergies and conditions as it doesn't all fit on my bracelet.

I started years ago using the over night emergency medical for the Girl Scouts used as an example to follow for emergency medical forms for all of us. One copy on each vehicle. I carry one for me in my wallet. It gets updated at each change. It is kept on the computer and can be printed for an appointment.

Now to answer your question. Yes put it on the form. It is a procedure you have had done. I also keep apporoximent dates listed on that form.
