Lennox 12acb30-5p Seer rating????

This is my current ac unit. Ser # is 5801F 12511. I'm coming up with 2001 as the year made. Is the seer rating 12 ?? It works fine, just trying to fiqure out if I really need to replace it. The furnace is from 1977 so that is going to be replaced.....
