e-mail link html info needed

I have an e-mail link on one of my websites and have it coded so that the subject line is already filled out. I would also like to have a template inside the message area that would ask for name, e-mail address, location, comments, etc. Is this possible and can someone tell me how to code it? I have searched for the answer, but so far haven't had any luck.
