Garage Door Opener Trouble

I have a 1/3 hp liftmaster screw drive that was installed by the builder. Appox 4 yr old.

All of a sudden this am it started to get loud like a grinding noise and the door does not open. I figured the gears might be stripped out but I noticed the long screw is turning. I can see the black piece moving inside the tube.

I loosen the trolley and manually opened the garage door and locked the trolley. I pushed the button and still making the loud noise and I can see the plastic slider move and hit the trolley and closed the garage door. Right before it reaches the floor it makes a free falling drop.

I was wondering what is wrong. I'm thinking it is the trolley part that is messed up or stripped out.

Any ideas?
