Craftsman garage door opener 1/2 hp

I have a Craftsman model 139.53627SRT that has worked great till now.The chain jumped off of the sprocket. I reinstalled the chain and adjusted to leave a little slack and it jumped off again. The door moves freely when unlatched. Any help would be appreciated.

This is generally caused by a bad bearing letting the sprocket lean forward and the chain jumps off. You can get the part at Sears or any opener service company. It is called a Gear and Sprocket assembly and the part number is 41C4220A. It should also come with instructions.

Changed the sprocket assy and the door works great!!
