90 year old concrete slab house and wood floors

Because of a powder post beetle problem :( I had to take up two wooden floors in my home. The floors were built as follows: sleepers nailed directly to concrete, no insulation, and then beech flooring put on top of the sleepers.
I'd like to know the best way (and best materials to use) when new wood flooring is installed.
What do you recommend for a vapor barrier? Insulation? I need to use sleepers, but would you recommend a plywood "subfloor" on top of them?
