wood transition thickness

In installing wood transitions from new tongue and groove flooring to older flooring at a lower level, how should the edge of the transition on the new floor end up? Should it be so flush so that any elevation change is not noticable? That might require planing of the transition to get the thickness just right.How is this usually handled?

An installer will look at what is there and then proceed with an appropriate transition. There is usually an elevation change when a higher floor meets a lower one.
