Cable TV Billing Question

I have 2 TV sets. I'm being billed for 2 cable boxes, which is fine. I'm also being billed for something called "Additional Set - Premium Service Fee", which I don't understand. If I have 2 cable boxes, isn't it assumed that I'm using one of them for an additional TV? I'm trying to find out the answer from my cable company, but I'm wondering if anyone has had questionable charges on their bill.
