Housewarming basket poem

I have misplaced a poem for a gift basket. I think I got it on this forum quite a while back. It had a candle for light, salt for spice and a loaf of bread and I can't remember what else. Does anyone have the poem? Thanks

When someone moves into our neighborhood I give them a basket containing salt, a bottle of wine and a loaf of home baked bread. The card reads:

Bread that you will never be hungry
Salt to recognize that life can be difficult
Wine for good health, good cheer and many friends

I also give them a card with all the important phone numbers (including my own) and garbage/recyclable pick up schedule.
