Thicken apple pie filling with tapioca or flour?

What is best for thickening the apple pie filling - tapioca, flour, or cornstarch? While I'm on the apple pie subject how do I keep the top crust from staying up like a dome while the filling has sunk and there is a big space between the filling and the crust? I've tried cutting bigger slits but hasn't helped.
