How do you make Smothered Pork Chops?

I brown my chops then drain the grease from the pan and throw a can of Cream of Mushroom soup and 3/4 can of water on top with lots of black pepper. Simmer 30 minutes. Serve with rice and peas.

It is so good, but I know there must be a way to get it just as good without the high sodium and fat in the canned soup. And I've tried the low fat, low sodium soups and did not like them. I also do not like the Golden Mushroom soup in this.
