But as a builder, when this has happened I've simply told the former to remove the damaged part back to an undamaged portion, then reform and repour it with rebar connecting the damaged to undamaged portions.

I've seen subdivision houses where this has been "repaired." It stands out like a sore thumb.

It needs to be replaced.

A pour is not the time to make repairs. Things like this can be repaired with no problem but it should be done completely separately from a slab pour (just like the entire rest of the foundation) and done so you or your guy can look at it and approve.

The slab should not be used to cover up something broken so they can patch what's left visible. That would be the new guy with a five gallon bucket, two gallons of mud scavenged from the washout pit, and a two inch stubby margin trowel!

They broke it, they fix it, it gets examined after setting, everyone moves on.
