Propane tank explosion

In the local news recently, a BBQ propane tank exploded right after the food was cooked. It did not say in the news the cause of the explosion. Makes me want to get rid of our gas BBQ. Does anyone know if this happens often and what could have caused it?

The valve on top of the tank turns the gas on and off. I can only guess the valve was not securly screwed into the tank. Might want to consider getting rid of the cars since 40,000 some odd peo[ple are killed in auto accidents.

Hose not tightened into the tank valve correctly. It leaked.
