Shower tile/ design questions

1. How far up the wall should the tile go? I am doing a 4' x 5' walk in shower, no doors, in a studio with cathedral ceilings. Ceiling height is 7.5' at lowest, not sure how high at the center, perhaps 9'. The hsower is toward the center of the space and I don't want tile all the way up. My husband is 5'10", and I want a hand shower on a sliding bar and a rain shower head. I assume the tile should be at least 6'6" to acco modate the drop of the rain showerhead ( is this making sense?) so my husband can wash his hair comfortably. Would 7' be better? Thoughts?
2. I am choosing between a 6" x 6" and a 12" x 24" tile, partly based on color and partly on size. I am afraid the huge 12 x 24 tile will look dated in 10 years or so. Do you think tile this large is faddish? Usually I dont care, but tile is harder to change than some things.
