Fear of caulk

Hi,
My current shower is a 36x36 1 piece acrylic unit. The new shower will have tile walls, but I can't decide what type of floor to use. I want to minimize the amount of visible caulk lines because they always mildew. DH says caulk has probably gotten better in the last 20 years, but I'm not convinced.
Do you grout tile walls right up to the shower pan, or is the caulk line visible?
Also, since I'm out of lurker mode, how much caulk is needed in a frameless shower door install? Or, should I use a framed door but tile right up to the frame? I'm tempted to go with a shower curtain, but I'd like to actually see my tiled shower.
