what happens when you remove a bathtub

What happens when you remove - and I mean completely remove- a bath tub? Is there (must there be ?) a capped access drain left in the floor? Or can it be capped off below the subfloor and then smoothly tiled over?

The existing garden tub in our MB, with tile steps surrounding it, uses up a lot of space, and it is NEVER used. I want to take out this monstrosity and put a make up table in its place. We will be taking up the existing tile, removing the garden tub, and then retile the floor. (all done by contractors/plumbers)
btw, yes, there is a shower stall that is already roomy and will remain in the MB :-) And, we have another tub in the hall bathroom if a guest really wants to take a bath.

But I'm worried about the tile floor - will it have to show a cap where the drain was? I don't want it to be unsightly or, ouch, stub my toe on it from time to time.
