Most likely you would need to cut the hole where you want it in the top of the assembly and cover the hole in the back that you aren't using with sheet metal. Your HVAC specialist should be helping you with this. The hole in the top would need an adapter plate to form a collar. You would need to plan the top hole so that it falls between joists. It might also have location limitations depending on the hood design. You would pretty much have to have the unit on site to know for sure what to do.

How does one locate a HVAC specialist? I live in Houston and have contacted several HVAC companies. So far, none do vent hoods.
