Frost Proof Bib Setting

One of our frost proof bibs has the large frost proof head AND a lower spigot. A handyman has used the large head to turn water on and off. Now I'm confused about how this setup should remain over the winter to prevent pipe damage.

Top head open or closed?

Lower spigot open or closed?
